GUARDIANSHIP

At Infinite Routes, we understand that the guardianship process can be daunting! Our team of social workers can help with overall support with getting started, as well as completing the initial sections of one of the documents required for this process, known formally as the Clinical Team Report (CTR). A CTR must be completed and signed by: 1) A licensed social worker, 2) a psychologist, and 3) a physician.

Our team includes licensed social workers with many years of combined experience. We offer guardianship services for caregivers of individuals who are about to turn 18, or individuals over the age of 18, who may require a guardian to assist with different areas of decision making in regards to health, safety, and self-care.

Service Overview

Our licensed Social Workers will:

  1. Provide information and resources to caregivers regarding the guardianship process.

  2. Review all necessary evaluations/records/assessments (e.g., Individualized Education Program, neuropsychological evaluation, psychological assessment, etc.).

  3. Spend time meeting with the individual in need of guardianship, providers, family members, and caregivers to complete their required sections in the Clinical Team Report.

  4. Complete their required sections and/or sign the Clinical Team Report in a timely manner.
